What Is the Bundled Care Program?
The Bundled Care program (also known as Integrated Funding Model) is a government-funded initiative that offers a seamless care experience following surgery. Rather than paying separately for hospital and rehabilitation services, the system funds your entire recovery pathway under one “bundle.”
That means:
- Your post-operative physiotherapy is covered if you’re referred through the Bundled Care program.
- Care is coordinated between hospitals and clinics, like ours, to ensure smooth recovery.
- You receive timely, goal-oriented rehab designed to get you back to daily life faster.
Who’s Eligible?
You may qualify for Bundled Care-funded physiotherapy if:
- You recently had a hip or knee replacement surgery.
- Your surgery was performed at a participating hospital (e.g., St. Joseph’s Health System, Hamilton Health Sciences, or another regional hospital involved in the Bundled Care network).
- You were referred to us through your hospital or surgeon as part of your recovery plan.
